By entering and using Weights by Willis, I acknowledge and agree to the following:

1. Assumption of Risk – I understand exercise involves risk.
2. No Supervision – This is a private facility.
3. No Lifeguard on Duty.
4. Medical Responsibility – I am capable.
5. Release of Liability – I release the gym.
6. Equipment Use – I accept responsibility.
7. Access & Security – I will secure the facility.
8. Personal Property – Not responsible for loss.
9. Surveillance – Premises may be monitored.
10. One-Time Waiver – Valid for future visits.
